Nursing Assistant at Adult Family Home Charged With Unprofessional Conduct (PDF)

The owner of Country Care Manor in Gig harbor was charged after she failed to maintain, staff or operate the facility according to regulations and provided substandard care to residents.

A Gig Harbor registered nursing assistant Connie K. Gaylord has been charged with unprofessional conduct.

Gaylord owned and operated an adult family home in Gig Harbor called Country Care Manor.

The Washington Department of Health said the Nursing Assistant Program charged the woman in February 2013 after she failed to maintain, staff or operate the home according to regulations and provided substandard care to residents.

The Department of Social and Health Services revoked her adult family home license and issued a preliminary finding that she had neglected vulnerable adults.

The Country Care Manor closed in October after Gaylord’s license was revoked.
